    This form to be used for all new accounts or changes to accounts within the district student information system.

    Application Approval FormThis form must be completed for any application, extension, software or website utilized in the district.  The following link contains those which have already been approved or denied:


    An account is required to search the Student Data Privacy Coalition's Database. If you are unable to locate the application on the SDPC site listed above, you are responsible for reading the terms and conditions and/or privacy policy for the application.  If the application states it utilizes targeted adverting or third party sharing, the application cannot be used by students and any applications received which violate these terms will not be approved.

    This form is to be used to request use of District Property offsite.
